References:Please bring names, addresses, and phone numbers of last three agencies, or employers for whom you have worked and three personal references including names, addresses, and phone numbers.
Documentation.
Driver's license, proof of automobile insurance.
Social Security Card

 A Level 2 finger-print background screening is required at a cost of  $54.25

Mandatory Professional Liability Insurance: Current cost approximatly is $48 ($500,000 - $1,000,000) or $81 (for $1,000,000 - $3,000,000 per year.
